General Description: provides coordination of services or targeted case management to adults and children with cognitive, emotional, physical, and developmental disabilities residing in residential or community settings in Ohio, Marshall and Wetzel Counties in West Virginia.
Requirements: 1. Bachelor’s degree in a human services field – social work, special education, psychology, counseling or licensed RN.
- Bachelor’s degree in a non-human service field and one year of experience in the MR/DD field.
